Click on the appropriate link prior to entering St. Mary\u2019s School.<\/p>\n\n\n\n<ol class=\"wp-block-list\"><li>Are you or your student experiencing fever greater than 100.4 degree Fahrenheit, new or worsening shortness of breath, new or worsening cough or difficulty breathing, new or worsening muscle or body aches, new or worsening fatigue, sore throat, congestion or runny nose, new or worsening headache, loss of smell or taste, diarrhea, nausea or vomiting?<\/li><li>Have you or your student been in close contact (within 6 feet for at least 10 minutes) with Covid-19 positive individual in the last 2 weeks?<\/li><li>Have you or your student been in&nbsp; close contact (within 6 feet for at least 10 minutes)&nbsp; with anyone experiencing symptoms of Covid-19.<\/li><li>Have you or your child travelled internationally in the last 2 weeks.<\/li><\/ol>\n\n\n\n<p><strong>\u00a0If you answered NO to ALL of the above questions,\u00a0<a href=\"https:\/\/forms.gle\/3fcnCr5evqmoGwtU6\">please click this link<\/a>\u00a0to complete the screening process.
